 In the new issue of Polytrauma journal, as usually, we present some recent scientific researches and practical follow-ups relating to actual problems of polytrauma and adjacent fields.

The Original Studies section opens this issue and presents a study of clinical and predictive value of extended inflammatory parameters of hematologic analysis in development of infection in patients with polytrauma.

Currently, one of the difficult tasks for the surgeon is selection of amputation level for the lower extremity with occlusion of iliac arteries. The section Clinical Aspects of Surgery offers to study the results of preservation of the knee joint, and the rate of mortality after transtibial amputations in presence of this problem. Another study is dedicated to use of ultrasonic cavitation technique for combined treatment of trophic ulcers of lower extremities at the background of chronic venous failure.

The current incidence of polytrauma achieves 14-15 %. Therefore, it is not a surprising fact that issues of fixation and treatment of various fractures in polytrauma are still important. It is confirmed by traumatologists-orthopedists in their studies of treatment of the injured ankle bone in polytrauma, and in researches of results of conversion osteosynthesis for treatment of patients with fractures of long bones of extremities. Moreover, the study of estimation and significance of condition of the capsule in surgical treatment of chronic instability of the shoulder joint is interesting.

Over the extended time intervals, the issues of arrangement of specialized medical care for patients with spine and spinal cord injuries show the medical and social actuality. So, the section Clinical Aspects of Surgery offers the analysis of lethal outcomes in patients with spine and spinal cord injury in the acute period. The authors of the study note that the modern surgical care for patients with spine and spinal cord injury with use of microsurgical techniques, and realization of early restorative treatment allow the significant reduction of total mortality in such patients.

A diagnostic study of estimation of time course of C1-esterase inhibitor (C1I) and its role in prediction of outcome of severe traumatic brain injury proves that high consumption of this protein in blood serum is the early predictor of a poor outcome of the disease.

The clinical follow-up is presented by a case of successful treatment of a patient with polytrauma and extensive traumatic detachment of the skin of the left leg, and by a case with arteriovenous fistula as a complication after total replacement of the knee joint.

The review article presents the modern potential of stromal vascular fraction of fat tissue in traumatology and orthopedics.

I hope that the materials of this issue will be actual and helpful for our wide audience. We wait for discussion of all interesting and unsolved issues in the pages of our journal.
